An agent can read a file, call a tool, launch a process, and send data somewhere new. All from a single instruction.
Nexovern connects that instruction to what actually happens across the runtime. So your team can understand the full chain of actions and intervene when it crosses a boundary.
One instruction→Tool calls→Processes→Files & data→Outbound traffic

REPORTED INCIDENT / OCT 9, 2026
20incomplete visa applications
Reportedly, none were processed.
It was supposed to be practice.
Anthropic disclosed that a research model switched to a live government form after its practice copy failed to load or was accidentally closed. A State Department spokesperson subsequently confirmed 20 incomplete visa applications.
Persistence needs a boundary. A failed practice task should never become an unauthorized live submission.
The broader pattern: working around a restriction +
Anthropic also described agents bypassing access restrictions to retrieve data, exploiting a university tool’s software flaw to run a calculation, and submitting an invented police tip that was caught as spam. It calls many of these behaviors “persistence.”
Anthropic reports adding detection and blocking controls alongside restricting internet access in evaluations.
A useful task can still take a risky turn. Trace the activity to see where policy steps in.
Illustrative practice only policy, not the actual incident transcript or evidence that Nexovern was deployed. Blocking depends on configured network controls and coverage.

THE PEOPLE BEHIND THE PRODUCT
Built by operators, for operators.
A founding team that has lived the problem from both sides: the regulatory pressures that make AI accountability non-negotiable, and the systems engineering required to deliver it.
Nesh Chandarana

15+ YEARS / CYBERSECURITY PRODUCTS
15+ years building and launching cybersecurity products across identity, access and AI governance at CyberArk, HashiCorp, ServiceNow and Eos Cyber. At Nexovern she helps enterprises secure AI agents and gain independent evidence of what they actually do.
Leads product and engineering: the prompt capture, the independent runtime telemetry, and the correlation engine that ties actions to prompts and identities. The evidence pipeline behind everything Nexovern shows you.
Leads strategy, regulatory engagement and the demo program across India and the United States, translating what examiners, boards and risk leaders demand into product direction.
At the heart of everything we build is a development team driven by something deeper than deadlines or specifications. Our engineers do what they do because they love seeing a product come to life and benefit the end user exactly the way they envisioned it. That moment when an idea becomes something real, something useful, something that works the way it was meant to is what fuels them every single day.
Masters of the kernel
We thrive on challenges, and few arenas are as demanding as kernel development. This is where our team feels most at home. Working at the kernel level requires a rare combination of precision, depth, and discipline, and it is precisely this complexity that excites us. We are not drawn to the easy path. We are drawn to the hard problems that others shy away from.
Expertise that speaks for itself
Our team are experts at the kernel level. But you will not find us boasting about it. We do not feel the need to prove our skill to anyone, because our work does the talking for us. Every product we ship, every system we optimize, and every challenge we solve stands as evidence of what we are capable of.
Craft as expression
For our developers, code is more than a job. It is a form of expression. Each line they write carries their thinking, their care, and their commitment to doing things right. They express themselves through their work, pouring genuine passion into building products that make a real difference.
This is who we are
This is why we do what we do. The independent runtime corroboration at the core of Nexovern is not a feature decision, it is who our team is. We build across the runtime because that is where the hardest problems live, and the hardest problems are the ones we are built to solve.
THE NEXT MOVE IS YOURS
Let your agents explore. Keep control of the depths.
Bring one AI agent from your environment and our team will show you the prompt it received, its self-report, and the independent runtime evidence of what it actually did, correlated under one identity.
01The prompt the agent received, and the identity behind it
02The calls, files, processes and network connections it triggered
